Creating a Delayed Button or Form on a Squeeze Page

This is a neat trick. Here's how to create an opt-in form or any sort of button appearing next to a video on a squeeze page. The only problem with the first one is the button form appears from the time you open the page. In the second example the button or form is triggered from the time you play the video.
